Is it actually CI/CD?
Deployment frequency, batch size, lead time. Many teams have sophisticated pipelines and are still running high-batch delivery with better dashboards.
What's the real failure distribution?
Outcome rate per run, rerun rate, DORA metrics. Flakiness is technical debt with compounding interest — it needs a policy, not a prayer.
Where is the feedback loop breaking?
Pipeline duration split into queue time vs. execution time. What belongs in CI, and what belongs in a separate, slower pipeline.
What does the pipeline mirror back?
Slow, fragile pipelines usually reflect architectural coupling, not config problems. Repo sprawl, pipeline zoos, cross-cutting controls that never made it past the README.
